小剂量阿糖胞苷,三尖杉酯硷和维甲酸治疗急性白血病

自1989年8月我们用小剂量阿糖胞苷(10mg/m~2q12h)、三尖杉酯硷(0.02—0.03mg/kg·d)和维甲酸(60～80mg/日)治疗9例10人次急性髓系白血病患者,其中M35例,M41例,M53例。5例M3及1例M4获完全缓解,1例M5临床症状及血象明显改善,但骨髓象未获缓解,改用DAT方案化疗后获完全缓解。另2例M5发生严重骨髓抑制,因颅内出血死亡。1例M3复发后再次用此方案化疗获第二次缓解。本组有效率为80％完全缓解率为70％。生存期2～21个月,临床及实验室观察表明小剂量诱导分化剂联合化疗既有诱导白血病细胞分化成熟作用,也有细胞毒作用,可提高单种诱导剂的疗效,是一种治疗急性白血病的可行方法。

TREATMENT OF ACUTE MYELOCYTIC LEUKEMIA WITH LOW DOSE ARA-C,HAR RINGTONINE AND ALL-TRANS RETINOIC ACID

9 of patients suffering from acute myelocytic leukemia were treatedwith low dose Ara-c(10mg/m2 q 12h).Haririgtonine;0.02- 0.03mg/kg/d) and al-trans retinoic acid (60- 80mg/d) for 10 courses since August 1989 including 5 cases M3, 1 of M4 and 3 of M5. Rve of M3 and one of M4 obtained complete remission (CR), one of M5 had more improvement in clinical sign and peripheralblood picture but no remission in the bone marrow. She obtained CR with DAT lastly. Two of 9 cases occurred on severe myelosup-pression and died of brain bleeding. 1 of M3 relapsed had second CR with same method The overall effect rate was 80%, CR rate 70% ,suvival period 2-21 month. The observation in clinical and laboratory showed that this therapy had two effects both inducing leukemia cells to differentiation and cytoxicity , it is more effect than single inducing agent for treating acute myelocytic leukemia.
