多元化心理护理在呼吸机相关性肺炎患者治疗后期撤机困难中的应用

目的:探讨多元化心理护理对呼吸机相关性肺炎(VAP)患者治疗后期撤机困难中的作用。方法:将38例VAP患者随机分为对照组和观察组各19例,对照组采用人工气道常规护理及常规鼻饲方法;观察组在常规护理的基础上着重加强多元化心理护理。结果:观察组撤机效果明显好于对照组(P<0.05);撤机后1周症状自评量表(SCL-90)评定中观察组强迫症状、抑郁、焦虑、恐怖、偏执、人际关系敏感6个症状因子的情况明显好于对照组(P<0.05,P<0.01);同时其临床肺部感染评分(CPIS)明显较对照组低(P<0.05)。结论:对患者脱机前进行耐心细致的多元化心理护理,有利于纠正VAP患者治疗后期撤机困难,促进患者早日康复。

Application of multi psychological care to the patients weaned off ventilator in the later stage of the treatment of VAP

Objective: To explore the effect of multi psychological care to the patients weaned off ventilator in the later stage of the treatment of ventilator-associated pneumonia(VAP).Methods: 38 VAP patients were randomly divided into an observation group and a control group(19 cases in each group).The patients in the control group were given conventional artificial airway care and routine nasogastric feeding and the multi psychological care was enhanced in the observation group on the basis of routine nursing care.Results: The effect of weaning from ventilator was obviously better in the observation group than the control group(P<0.05);the six symptom factors like obsessive-compulsive symptoms,depression,anxiety,horror,paranoia and sensitive interpersonal relationship in the assessment of SCL-90 were evidently fewer and milder in observation group than the control group after weaning from ventilator for one week(P<0.05,P<0.01);at the same time the clinical pulmonary infection score(CPIS) was significantly lower in the observation group than the control group(P<0.05).Conclusion: The patient and meticulous multi psychological care is effective to solve the tough problems when the patients were weaned off ventilator in the later stage of the treatment of VAP and promote early recovery of the patients.
