乳腺肿块超声图像特征与病理对照分析

目的：探讨肿瘤超声特征与病理改变的关系,提高乳腺肿瘤超声定性试诊断的符合率。方法：本文对177例乳腺肿瘤患者进行二维超声检查,部分患者同时进行了彩色多普勒血流检测,并分别与病理对照。结果：良、恶性乳腺肿瘤的声像图有明显差异,主要表现在两者边界、包膜、形态、回声分布、后方声衰减、钙化灶有无及形态等方面;乳腺肿块以实性低回声型多见,混合型次之,等回声仅6例,暗区内伴有实性乳头型1例,强回声1例;恶性肿瘤的分型无特异性;硬癌、髓样癌液化坏死多见;对较早期和/或体积较小的乳腺癌和不典型乳腺纤维腺癌的鉴别配合彩色多普勒血流检查,可提高乳腺肿瘤定性诊断的敏感性和特异性。结论：通过超声图像特征,为分析乳腺肿瘤性质提供了重要信息。

Comparative Analysis of Ultrasonographic Features of Breast Mass and Its Pathologic Examination

Objective To investigate the correlation of ultrasonographic features of breast tumor and its pathologic changes.Methods 177 patients of breast tumor were examined by two-dimension ultrasonography,some of them were detected by colour Doppler imaging and compared with the pathological examination as well.Results There were significant differences in the acoustic images between benign breast tumors and malignant ones .They were mainly manifested as that whether or not there were clear border,capsule,morphologic change,echo distribution,and calcific focus ear.Breast mass as were mostly presented as solid and hypoechoic,and then the mixture form followed,6 cases were as iso-echo,1 case was shoused as solid papilloma in area opaca,1 case was as hyperechoic.The typing of malignant breast carcinoma showed no specificity:Colliquative necrosis has often seen in medullary carcinoma and sirrhous carcinoma.To those breast carcinoma in the early stage relatively smaller in size,its differential diagnosis with breart fibroadenoma should be combined with the examination of CDII,and thus increased the sensitivity and specificity of guantitative diagnosis of breart tumor.Conclusion The ultrasonogram feature could be used as an important reference for analyzing the property of breast tumor.
