彩色多普勒超声造影在肝脏占位性病变中的表现及鉴别诊断

目的 研究肝脏局灶性实质病变的彩色多普勒血流成像(CDFI)超声造影表现和规律。方法 对112个肝实质占位性病变进行了CDFI超声造影检查,其中良性病变34个,恶性病变78个,比较注射造影剂前后,病变内CDFI显示血流的变化。造影前后超声诊断符合率以手术病理诊断为对照。结果 造影前后良性病变血流显示率分别为44.1％(15/34)和55.9％(19/34)(P<0.05),恶性病变分别为67.9％(53/78)和93.6％(73/78)(P<0.01)。造影后各种肝脏疾病血流信号有不同增强表现,恶性病变增强幅度明显高于良性病变。诊断符合率由造影前的67.8％(76/112)提高至造影后的95.5％(107/112)。结论 CDFI肝肿瘤超声造影可以明显提高肝肿瘤的血流显示率及其诊断准确性。

Diagnosis in Focal Hepatic Lesions with Contrast- enhanced Color Doppler Sonography

Purpose To investigate the diagnosis in focal hepatic lesions with contrast-enhanced color Doppler sonography. Methods One hundred and twelve patients underwent examination with CDFI be-fore and after intravenous injection of Levovist. Results Conventional CDFI showed intratumoral arterial flow in 15 (44.1%) benign hepatic lesions and 53(67. 9% ) in malignant ones. After contrast, 19(55. 9% ) benign lesions and 73(93. 6% ) malignant lesions exhibited intratumoral arterial flow. Liver tumors of differ-ent pathological types and their own appearence of blood flow on contrast sonography. Time-intensity changes in malignant tumors were stronger than in benign tumors. The accuracy rate of diagnosis was 67.8% (76/112) before contrast and 95.5%(107/112) after contrast with a distinguished improvement. Conclusions Enhanced CDFI can improve evaluating the vascularity of focal hepatic lesions and aid in differential diagnosis between benign and malignant lesions.
