| 摘 要： | 目的 探讨腔内激光(endovenous laser treatment,ELT)联合局部缝扎治疗下肢静脉性溃疡(venous leg ulcer,VLU)的临床疗效.方法 VLU患者11例11条肢体,从内踝处穿刺或从腹股沟处切口植入激光光纤消融大隐静脉主干;对小腿侧支静脉用套管针穿刺置入光纤行消融术,若失败改用经皮皮内环形缝扎术.距离溃疡边缘约1 cm,用2-0 Prolene缝线环形间断缝扎一圈.结果 术后11条患肢内踝处色素沉着减轻.术后12～25 d(平均18 d)小腿溃疡愈合.无皮肤坏死、切口感染、深静脉血栓形成等并发症.随访3个月以上,无溃疡复发.结论 ELT联合局部缝扎是一种操作简单、微创、疗效确切治愈下肢静脉性溃疡的方法,对有交通静脉功能异常的溃疡患者也是一种有效的处理手段.
