随州市中心医院精神科会诊263例分析

目的：了解本院精神科会诊的临床特点。方法：回顾性分析本院3年间精神科会诊的263例临床资料。结果：会诊病例中，男105例、女158例，在〈19、20～39、40～59、≥60岁各年龄段中，会诊率（％）分别0．21、0．28、0．33、和0．18；紧急会诊57例，普通会诊206例，会诊总数305次，会诊率0．39％；会诊科室占62．5％，神经外科59例次，消化内科53例次、心血管内科48例次、中医科34例次、神经内科24例次、内分泌科18例次、儿科15例次、其他27例次；会诊原因，无明确器质性基础的躯体症状而疑有精神疾病者125例，有明确躯体疾病并出现精神障碍者92例，原有精神疾病又患躯体疾病者46例；会诊诊断脑器质性及躯体疾病所致精神障碍109例、神经症89例、精神分裂症31例、情感性精神障碍22例、应激障碍18例、其他精神障碍12例；会诊后转科治疗者86例、不同意转科者38例，未转科医嘱中，使用中小剂量抗精神病药71例、抗抑郁剂42例、抗焦虑剂59例、患者拒绝会诊意见5例，医嘱执行率为84．9％，治疗总有效率为80．3％。结论：本院须尽快弥补各科重视精神卫生的程度不一、大多临床医生缺乏识别精神症状的能力和科间协作力度不够等不足。

Analysis of 263 cases in Suizhou Central Hospital psychiatric diagnosis

Objective:To investigate the clinical characteristics of hospital psychiatric consultation.Methods:A retrospective study was conducted three years of psychiatric diagnosis of 263 cases of clinical data.Results: Consultation to target young women;57 cases of emergency consultations ordinary 206 cases,a total of 305 times,the diagnosis rate 0.39%;diagnosis sections accounted for 62.5%,neurosurgery 59 times,gastroenterology meeting of 53 cases,48 cases of cardiovascular medical meeting,34 cases of the medical meeting,24 cases of neurology at times,endocrine 18 cases,15 cases of pediatric;for the diagnosis of brain and body to diagnose disease organic mental disorder caused 109 cases,89 cases of neurosis,schizophrenia in 31 patients,22 patients with affective disorder,stress disorder in 18 cases and 12 other cases.Section 86 cases diagnosis later,38 cases do not agree to change,not to change medical advice,use small dose antipsychotic 71 cases,42 cases of antidepressants,anti-anxiety 59 cases,patients refuse to diagnose views five cases,the doctor's order rate was 84.9%,the total effective rate of 80.3%.Conclusions: The importance of mental health hospital as soon as possible to make up branches in varying degrees,most clinicians,the lack of recognition of the ability and psychiatric symptoms such as lack of enough collaboration.
