肝胆管结石病并发肝胆管癌CEA表达的研究

作者采用高灵敏度的免疫组化ＳＰ法对９例胆管癌（ＣＣ），１００例肝胆管结石病（ＨＬ）及１５例正常肝胆组织的癌胚原（ＣＥＡ）表达进行，结果发现正常胆管不表达ＣＥＡ；９例ＣＣ的ＣＥＡ阳性率为１００％，呈阳性较高的非规则性分布；１００例ＨＬ中伴异型曾生的有２７例，其ＣＥＡ阳性率为５９．３％。呈极性分布，表明组织ＣＥＡ的检测将有助于ＨＬ并发ＣＣ的诊断，对对于ＨＬ胆管增生性病变性质的判别具有一定的参考价值。

A study on CEA expression in patients with hepatolitheiasis and cholangiocarcinoma using immunohistochemicalmethod

An immunoenzymologic method(S-P method) was applied to the identification of carcinoembyronic antigen(CEA)on paraffin sections of bile ducts,in the normal liver,hepatolithiasis,and cholangiocarcinoma.It was found that the CEA did not present in normat bile ducts but usually revealed a weaker reaction in the apical border of biliary duct epithelium with a positive rate of 59.3% in hepatolithiasis with atypical epithelial hyperplasis,which might be the result from the chronic cholangitis.Whereas in the most bile duct cholangiocarcinoma(CC)tissue,the CEA showed a stronger reaction and irregular cytoplasm localization.Intensity of the CEA reaction and its localization pattern were colsely related to the pathological type of the tumor.In papillary adenocarcinoma,the CEA exhibited a weaker reaction on membrane,membrance but in ductal adenocarcinoma,its reaction was stronger and more frequently preseented in the cytoplasm.It suggested that CEA detection might be useful for the diagnosis of CC which complicated from hepatolithiasis and the CEA localization pattern were more important than its recation intensity.
