CRP检测在妊娠合并急性泌尿系感染中作用探讨

目的探讨CRP在妊娠合并急性泌尿系感染中的作用。方法检测2005年6月～2006年6月32例妊娠合并急性泌尿系感染病例的CRP。其中妊娠合并急性肾孟肾炎和妊娠合并急性膀胱炎各16例。选择同期16例健康孕妇作对照组，采用免疫比浊法检测CRP（用Twbox Plus检测仪）。结果妊娠合并急性肾孟肾炎治疗前发热天数与CRP值正相关（r^2=0．528 P＜0．01）。CRP为40mg／L时，诊断急性肾孟肾炎敏感性为0．92，特异性为0．97。妊娠合并急性肾孟肾炎CRP平均值为78mg／L，急性膀胱炎组CRP值仅为10mg／L，正常孕妇CRP平均值为6mg／L。急性肾孟肾炎组明显高于急性膀胱炎组，差异有极显著性（P＜0．01）。结论CRP是妊娠合并急性肾孟肾炎病情观察的指标，CRP值与患者治疗前发热时间长短有关。在鉴别妊娠合并急性肾孟肾炎和急性膀胱炎中，CRP是一个特异性和敏感性较好的指标。

Investigate role of CRP in acute urinary tract infection during pregnancy

Objective: To investigate role of CRP in acute urinary tract infection during pregnancy.Methods:CRP was determined in serum from 32 consecutive patients with acute pyelonephritis(16) and acute cystitis during pregnancy(16) in 2005.6~2006.6,respectively,Healthy pregnancy blood donors served as control.Analysis of CRP was performed by turbidimetry with Turbox Plus.Results:Positive correlations were correlations was found between CRP lever and fever duration before admission(r2=0.582,P<0.01).CRP value(P<0.01) was significantly higher in the pyelonephritis group than in cystitis group.Efficiencies of the tests for differentiating between pyelonephritis and uncomplicated cystitis was 0.97,cutoff levels was set at 40mg/L sensitivities was 0.92.Conclusion:CRP is one better item to observe at acute pyelonephritis during pregnancy,CRP value is relative to fever period before treatment.CRP have sensitivity and efficiency in the discrimination between pyelonephritis and cystitis during pregnancy.
