腹腔镜手术与开腹手术治疗休克型输卵管妊娠的临床分析

目的:探讨腹腔镜手术治疗休克型输卵管妊娠的可行性。方法:对我院不同手术方法治疗的73例输卵管妊娠伴低血容量休克患者进行回顾性分析。其中腹腔镜手术41例,开腹手术32例,对两组病例的术前病史、术中情况和术后结局作比较。结果:73例患者全部治疗成功,无并发症发生,腹腔镜组无中转开腹。两组患者在术前病史、血压、腹腔内出血量、手术方式等方面无显著差异;而手术时间、术中出血量、术后肛门排气时间、住院天数等方面腹腔镜组明显优于开腹组(P<0.01)。结论:应用腹腔镜手术治疗休克型输卵管妊娠是安全可靠的,且具有术后肠道功能恢复快、住院时间短等优点。

Clinical analysis of the treatment of oviduct gestation with hypovolemic shock by laparoscopy or open surgery

Objective:To explore the feasibility of treating tubal pregnacy with hyporolemic shock by laparoscopy.Methods:Restrospectively analysed 73 cases who had tubal pregnacy with hypovolemic shock and underwent different operations.41 of them were treated by laparoscopy and 32 by open surgery.The preoperative case history,intraoperative condition and post operative outcome were compared between the two groups.Results:All cases were operated successfully without complications.No case in laparoscopy group was converted to open surgery.The preoperative case history,blood plessure,hemorrhage volume inside abdomen and operation method between the two groups had no significant difference.In the aspects of operation time,blood capability in operation,time of exhaust after operation,length of hospital stay,and so on,laparoscopy group were significantly better than open group(P<0.01).Conclusions:It is safe and credible that tubal pregnacy with hypovolemic shock be treated by laparoscopy,which has more merits including recovery of intestine function rapidly after operation and shortening the length of hospital stay.
