99TcmO4-及99Tcm-MIBI联合显像判定甲状腺结节性质

目的评价甲状腺99TcmO4-及99Tcm-MIBI显像在甲状腺结节性质判定上的价值。方法64例患者(其中甲状腺结节61例有病理结果,甲状腺癌术后3例),常规99TcmO4-甲状腺平面显像后,隔日行99Tcm-MIBI显像。结果17例甲状腺癌患者99TcmO4-显像冷结节14例,温结节3例,热结节0例。甲状腺冷结节的患者中99Tcm-MIBI显像诊断为恶性肿瘤的灵敏性为100%,特异性为53.13%,准确性为67.39%。3例甲状腺癌术后复发患者,99Tcm-MIBI显像均为阳性。结论99TcmO4-及99Tcm-MIBI显像对判定甲状腺结节性质及了解甲状腺癌术后有无复发有一定价值,可以避免部分患者不必要的手术。

Combining 99TcmO4- and 99Tcm-MIBI thyroid scintigraphy in thyroid malignant and benign nodules

Objective To assess the role of ~ 99 Tc~mO_4~- and ~ 99 Tc~m-MIBI thyroid scintigraphy in distinguishing between benign and malignant thyroid!nodules.Methods A total of 64 patients on the (99m)Tc-pertechnetate and (99m)Tc-methoxyisobutylisonitrile(MIBI) scan were included in this study,61 had thyroid nodules with pathology and 3 had recurrent thyroid carcinomas.Results The cases of 17 patients with thyroid malignant lesions,cold nodules,warm nodules and hot nodules on (99m)Tc-pertechnetate scan were 14,3 and 0 respectively.The sensitivity,specificity and accuracy of ~ 99 Tc~m-MIBI imaging were 100%,53.13% and 67.39% respectively.All the 3 patients with recurrent thyroid carcinomas showed positive imaging on ~ 99 Tc~m-MIBI scan.Conclusions Combining ~ 99 Tc~mO_4~- and ~ 99 Tc~m-MIBI thyroid scintigraphy can assess the malignant and benign of thyroid nodules,avoid some unnecessary operation.
