烧伤致胸壁及上肢粘连挛缩的整复(附8例报告)

目的　对 1990～ 1999年收治的 8例烧伤后腋窝、胸壁、上肢严重瘢痕挛缩的病人修复。方法　采用胸部、肩背部局部舌形皮瓣、局部筋膜瘢痕瓣、肩胛皮瓣和全厚皮片修复。结果　术后肩关节功能恢复 ,上肢均可上抬 110°以上 ,内收、外展功能良好 ,随访病人腋窝无继发性挛缩。结论　烧伤后胸壁、上肢粘连病人的整复重点仍在腋窝 ,以皮瓣修复为首选。肩胛皮瓣因供区隐蔽、血管解剖恒定、切取方便等优点而多被选用

Plastic treatment of contracture adhesion between chest wall and upper extremities after burn:A report of 8 cases

Objective Eight burned patients associated with severe scar contracture at axilla,chest wall and arms,which admitted into hospital from 1990 to 1999,were subjected plastic and repair surgery.Methods Using tongue shaped skin flaps from local chest,shoulder and back,scar flaps of local fascia,skin flaps of scapula,and all thickness skin flaps to repair the injury.Results After plastic and repair surgery function of shoulder joints were restored;upper extremities elevated up to more than 110°,and its functions of internal flexure and external extension were excellent,no secondary contracture found during follow ups.Conclusions For burned patients associated with adhesion of chest wall and upper extremities,the important caution point still were at axilla site,first choice would be flap repair,in which due to its such advantages as hidden blood support region,constant vascular anatomy of skin flaps of scapula,and casy to cut pick off,the skin flaps should be often applied.
