环氧化酶-2抑制剂对急性胰腺炎大鼠继发全身炎症反应保护作用的实验研究

摘    要:目的探讨环氧化酶-2(cyclooxygenase-2,COX-2)抑制剂对胰腺炎过程中免疫异常的调整作用机制。方法SD大鼠36只,随机分成对照组、胰腺炎组、美洛昔康(莫比可)和塞来昔布(西乐葆)治疗组。分别于术后第2,6,24小时三个时段各处死每组大鼠3只,观察大鼠的一般情况和腹水发生情况,测定腹水量。收集各时段血液标本检测白细胞计数、血液淀粉酶值,用酶联免疫吸附法(ELISA)检测IL-6、IL-10水平。切取各时段大鼠胰腺组织称湿重后行病理学检查。结果莫比可和西乐葆治疗后第24小时,除胰腺湿重外,其余各项指标均较胰腺炎组明显好转,具有统计学意义。结论COX-2抑制剂通过阻断花生四烯酸代谢产物的产生,减轻胰腺组织炎症反应程度,同时调整致炎和抗炎细胞因子水平,达到低水平平衡状态有效改善胰腺炎大鼠的预后。

Experiment study of the protective effect of inhibitor of cydooxygenase-2 on the systemic inflammatory response of rat in acute pancreatitis

Abstract:Objective To investigate the modulating mechanism of inhibitor of cyclooxygenase-2 which interfere with the abnormity of immunity during the course of pancreatitis. Methods Thirty six Sprague-Dawley(SD) rats were randomly divided into four groups: control groups,pancreatitis groups,groups treated with Mobic and groups treated with Celecoxib. The rats in four groups were individually killed at 2,6,24 hours after operation. And the ordinary situation and ascites of rats were observed. The volume of ascites were collected and measured. Blood samples were taken for measurement of blood routine counting,blood amylase,IL-6 and IL-10. The tissues of pancreas in different time were taken,and the wet weights and pathologic examination were also tested. Results Compared index in group 3 and group 4 to that in group 2 at the 24th hour after treatment,every index improved significantly (P<0.05) except the wet weights of pancreas in this experiment. Conclusion Cyclooxygenase-2 inhibitor can blockage the product of arachidonic acid,relieve the degree of inflammation reaction in pancreas and ameliorate the prognosis in pancreatic rat. Its protective effect may be achieved through rectifying both the level of pro-and anti-inflammatory cytokines and keeping immunologic balance in a lower level.
