妊娠前空腹血糖对妊娠期糖尿病的影响

目的:探讨妊娠前空腹血糖水平(FBG)对妊娠期糖尿病(GDM)的影响。方法:采用前瞻性队列研究方法,选取2006年至2017年开滦职工健康体检并在体检后住院分娩的育龄妇女1099例,根据妊娠前空腹血糖水平进行三分位分组,Logistic回归分析GDM发生的影响因素。结果:GDM患病率为11.46%。单因素及多因素线性回归结果均显示,妊娠前空腹血糖水平与妊娠晚期空腹血糖呈正相关(P<0.05)。单因素及多因素Longistic回归分析显示,妊娠前空腹血糖水平第三分位组是GDM的危险因素(OR=1.93,95%CI为1.15~3.25,P<0.05)。年龄(OR=1.08,95%CI为1.01~1.15,P<0.05)及肥胖(OR=1.38,95%CI为1.03~1.85,P<0.05)均为GDM的危险因素。结论:妊娠前空腹血糖水平增高是GDM的危险因素。

Effect of fasting blood glucose before pregnancy on gestational diabetes

Objective:To investigate the effect of fasting plasma glucose(FBG) level before pregnancy on gestational diabetes mellitus(GDM).Methods:A prospective cohort study was conducted to investigate the cohort of women of childbearing age who were enrolled in the 2006~2017 health checkup and were hospitalized after the physical examination(n=1099).Tripartite grouping according to the level of fasting blood glucose before pregnancy,the influencing factors of GDM in gestational diabetes mellitus were analyzed by logistic regression.Result:The prevalence of gestational diabetes was 11.46%.Univariate and multivariate linear regression results showed that fasting blood glucose levels before pregnancy were associated with fasting blood glucose in late pregnancy(P<0.05).Univariate and multivariate Logistic regression analysis showed that pre-pregnancy fasting blood glucose were risk factors for GDM(OR 1.93,95%CI 1.15~3.25,P<0.05).Age(OR 1.08,95%CI 1.01~1.15,P< 0.05) and obesity(OR 1.38,95%CI 1.03~1.85,P<0.05) were all risk of GDM.Conclusion:Increased fasting blood glucose levels before pregnancy are risk factors for GDM.
