摘    要:目的了解遂宁市中心医院重症监护病房(ICU)内患者及其环境分离的鲍曼不动杆菌(AB)耐药性与同源性,为预防AB医院感染提供科学依据。方法检测2017年3月至2018年7月我院ICU患者医院感染检出的所有AB共计44株(患者组)及同期其环境及工作人员手分离出的所有AB共计35株(环境组)的药敏结果,采用肠杆菌科基因间重复序列-聚合酶链式反应(ERIC-PCR)技术进行同源性分析,采用χ2检验比较两组AB耐药情况,聚类分析判定其同源性。结果患者组AB检出率为9.98%(44/441),环境组检出率为11.95%(35/293)。患者组对各类抗菌药物耐药率均较高,其中最低的为复方新诺明(63.64%),环境组耐药率最低的为妥布霉素(37.14%)。比较两组菌株耐药率,头孢曲松(χ2=8.663;P=0.013)、妥布霉素(χ2=11.486;P=0.001)、庆大霉素(χ2=6.767;P=0.034)、左氧氟沙星(χ2=14.472;P=0.001)差异有统计学意义。两组AB可分为...

Drug resistance and homology of Acinetobacter baumannii isolated from intensive care unit from Suining Central Hospital

Abstract:To understand the drug resistance homology of Acinetobacter baumannii(AB) isolated from intensive care unit(ICU) of Suining Central Hospital, and to provide scientific basis for preventing nosocomial infection of AB. Methods Antimicrobial susceptibility tests of 44 strains AB of nosocomial infection(patient group) and all AB 35 strains isolated from ICU environment and staff(environment group) were done during the same period from March 2017 to July 2018 in ICU. Homology were analyzed by Enterobacteriaceae intergenic repeat sequence-polymerase chain reaction(ERIC-PCR). The resistance of AB in the two groups was compared by Chi-square test, the homology was determined by cluster analysis. Results The detection rate of AB was 9.98%(44/441) in patients group and 11.95%(35/293) in environmental group. The drug resistance rates of all kinds of antibacterial drugs were high. Among the drugs, the drug of lowest resistance rate was compound neotamine(63.64%). In environmental group, the lowest was tobramycin(37.14%). The resistance rates of the two groups of ceftriaxone(χ2=8.663; P=0.013), tobramycin(χ2=11.486; P=0.001), gentamicin(χ2=6.767; P=0.034) and levofloxacin(χ2=14.472; P=0.001) were statistically significant. Two groups of AB can be divided into eight different types, expressed by A-H, of which A, D and G were the main epidemic strains. Conclusion The AB resistance rate is high and homologous in ICU patients and environment.
