中国精神分裂症患者暴力行为影响因素的Meta分析

目的 探讨中国精神分裂症患者暴力行为的影响因素。方法 检索CBM、CNKI、PubMed、Medline等9个中英文数据库。根据文献纳入排除标准筛选文献并进行质量评价、资料提取，采用RevMan 5.3软件进行Meta分析。结果 共纳入27篇文献，研究对象共计3 797例（暴力组1 353例，非暴力组2 444例）。性别、配偶、学历、既往暴力史、治疗依从性、幻觉、妄想、易激惹、居住地合并OR值及其95%CI分别为1.43(1.06～1.93)、0.76(0.61～0.95)、1.37(1.07～1.75)、3.83(2.54～5.79)、0.32(0.26～0.39)、2.21(1.62～3.01)、2.90(2.08～4.03)、4.15(2.78～6.18)、1.75(1.20～2.54)。亚组分析提示三大经济圈组和非三大经济圈组的影响因素不尽相同。结论 精神分裂症患者的暴力行为与多种因素有关。部分因素与暴力行为的关联可能受经济影响，应对不同地区采取有针对性的精神卫生管理措施，预防其暴力行为的发生。

Influencing factors of violent behavior in schizophrenics in China: a meta-analysis

To evaluate the influencing factors of violent behavior in schizophrenics in China, and to provideevidence for developing mental health policy in the future. Methods Relevant studies published from database set -up toFebruary 2020 were retrieved from CBM, CNKI, PubMed, Medline, etc. Quality evaluation of the included literatures wasconducted according to the inclusion and exclusion criteria, followed by data extraction and Meta -analysis with softwareRevMan 5.3. Results A total of 27 studies involving 3797 subjects (1353 violence and 2444 non-violence) were included. Thepooled OR and its 95%CI of each influencing factor were: male OR=1.43 (95%CI: 1.06-1.93), spouse OR=0.76 (95%CI: 0.61-0.95),education OR=1.37 (95%CI: 1.07-1.75), history of violence OR=3.83 (95%CI: 2.54-5.79), therapy compliance OR=0.32 (95%CI:0.26-0.39), illusion OR=2.21 (95%CI: 1.62-3.01), delusion OR=2.90 (95%CI: 2.08-4.03), irritability OR=4.15 (95%CI: 2.78-6.18),urban residence OR=1.75 (95%CI: 1.20-2.54). The results of subgroups analysis showed that influencing factors of violentbehavior in schizophrenics in the two subgroups (Three Economy Zones subgroup and None Three Economy Zones subgroup)were not the same. Conclusion Violent behavior in schizophrenics can be caused by many factors. The associations betweensome factors and violent behavior are affected by the economy. More attention should be paid to mental health managementin these cities.
