骨盆巨细胞瘤的影像学表现及病理特点对照研究(5例报道并文献复习)

目的探讨骨盆巨细胞瘤的影像学表现。方法回顾性分析5例具有完整的X线、CT检查资料,并经手术或骨穿刺病理证实的骨盆巨细胞瘤患者。结果5例患者中2例在CT定位下骨盆穿刺活检。病检结果:1例多发灶(有3个瘤灶),另4例为单发灶。4例病理分型为1级,1例为2级。CT显示骨质破坏区内皂泡状骨嵴及单层或多层骨包壳影,为巨细胞瘤的典型表现。当骨包壳有吸收变薄、中断或消失,考虑局部有恶变。结论CT检查能够清晰显示巨细胞瘤瘤灶内骨嵴及周围骨包壳的细微改变,CT对巨细胞瘤病灶显示明显优于平片。分析骨盆巨细胞瘤的影像学表现及病理特征有助于骨盆骨肿瘤的诊断。

The Correlation Research of Imaging Manifestation and Pathological Features on Giant Cell Tumor of Pelvis Bone: a Report of 5 Cases and Literature Review

Objective To explore the imagining manifestation with pathological features on giant cell tumor of pelvis bone patients. Methods 5 cases confimed by surgical bone puncture findings and pathology findings the data of x-ray and CT scan were analyzed retrospectively. Results 2 cases in 5 cases giant cell tumor of pelvis bone, positioning in the machine under the needle biopsy were diagnosed by pathology, one case 3 tumors） were single-shot＇s, 4 cases get rich muchly, 4 cases of pathology is diagnosed into 1 grade, one case of pathology is diagnosed divided into 2 grade. Giant cell tumor had typical imaging feature such as soap bubble and unilayer or multilayer involucrum in destruction of bone, canceration was considered when involucrum of bone was thinning, discontinuation or disappearance. Conclusion For clear display on tiny change of bone cristae and involuclum in giant cell tumor CT scan have more advantages than x-rays, is helpful to diagnose pelvis bone tumor. Analysis of giant cell tumor of the pelvis of the imaging findings and pathological features, contribute to the diagnosis of pelvic bone tumors.
