电化学发光免疫法定量检测乙肝病毒的临床应用

摘    要:目的 了解电化学发光法(ECL)检测乙肝病毒标志物的应用价值。方法 用ELISA法和ECL法平行测定360例疑似乙肝病人血清乙肝五项标志物,并对ELISA法检测为阴性而ECL法检测为低浓度HBsAg的标本,再用微粒子酶免分析法(MEIA)进行复测并分析。结果 ELISA法和ECL法对HBsAb、HBeAg、HBeAb三项标志物的检出率无显著性差异,但对HBsAg的检测,ECL检出率明显高于ELISA法(P〈0.05)。ELISA检测为阴性而ECL法检测为低浓度HBsAg的标本,用MEIA法复测均为阳性。结论 ECL法检测血清乙肝标志物的灵敏度和自动化程度均比ELISA法高,同时可动态观察疗效和监测病情。

Abstract:Objective To understand the theory, the advantage and the clinic application of HBV determined by electrical chemiluminescence immunoassay (ECL). Methods Five types of HBV in the serum of 360 cases have been determined directly by ELISA and ECL, the samples which are negative determined by ELISA however are positive with low concentration of HBsAg should be determined again by MEIA method. At last the results determined by ELISA and ECL are processed by one - to - one X2 test with SPSS 10.0 statistics software. Results The result is that the rate of detection of HBsAb, HBeAg and HBeAb are have no distinct difference determined directly by ELISA and ECL. but the rate of detection of HBsAg determined by ECL is higher than that determined by ELISA ( P < 0.05) , and the rate of detection of HBcAb is have distinct difference (P < 0.01). The results determined a-gain by MEIA are all positive of HBsAg. Conclusion The degree of sensitive and automation determined by ECL are higher than that of ELISA, at the same time the curative effect of patients is be easily dynamic observed and the state of illness is also be easily inspected by the method of ECL.
