| 摘 要： | 目的探讨多药耐药基因1（Multidrug-resistance Gene1,MDR1）C3435T位点单核苷酸多态性（Sin-gle Nucleotide Polymorphism,SNP）及其相应的蛋白产物-多药耐药蛋白（P-gp）在乳腺癌患者中表达的临床意义。方法收集宁夏医科大学附属医院154例乳腺癌患者的术后石蜡包块及相关临床及病理资料,应用免疫组化方法（IHC）检测乳腺癌组织中P-gp的表达状态,评估P-gp在乳腺癌中的表达及其临床意义;并随机抽取其中60例石蜡标本采用聚合酶链反应-限制性片段长度多态性方法分析（Polymerase Chain Reaction-Restriction FragmentLength Polymorphism,PCR-RFLP）各基因型,评估MDR1基因C3435T位点多态性与P-gp表达的关系。结果①P-gp阳性组化疗后白细胞和血红蛋白的Ⅰ-Ⅳ度下降率分别为79.44%和43.93%,均低于P-gp阴性组化疗后白细胞和血红蛋白的Ⅰ-Ⅳ度的下降率95.74%及63.83%,差异均有统计学意义（P〈0.05）;②MDR1C3435T位点野生纯合子TT基因型的P-gp阳性表达率4%明显低于突变后的CC＋TC基因型的P-gp阳性表达率96%,差异有统计学意义（P〈0.05）。结论 P-gp有望成为化疗用药的指南及预测化疗后出现血液毒性的指标;MDR1C3435T位点突变后的基因型可能更易导致其蛋白产物P-gp表达水平的变化。
