ACE基因多态性和吸烟与缺血性脑卒中关系的研究

摘    要:
目的探讨血管紧张素转换酶(ACE)基因多态性和吸烟与缺血性脑卒中(IS)患病的关系。方法应用聚合酶链反应(PCR)和变性高效液相色谱(DHPLC)技术筛查454例IS患者(病例组)和334例非IS患者(对照组)的ACE基因的多态分布,采用非条件logistic回归模型分析基因型、吸烟情况与缺血性脑卒中患病的关系。结果携带有DD基因型和D等位基因的吸烟群体患缺血性脑卒中的相对危险度分别为1.992和1.679;而携带有Ⅱ基因型的吸烟群体患缺血性脑卒中的相对危险度则为0.523。相反,非吸烟IS患者的ACE基因的各基因型和等位基因频率的分布与对照组相比,均无显著性差异(P〉0.10)。结论携带有D等位基因的吸烟群体容易患缺血性脑卒中,但携带有I等位基因的吸烟群体不容易患缺血性脑卒中,ACE基因与吸烟在缺血性脑卒中的发病过程中存在协同作用。

Abstract:
Objective To investigate the correlation of polymorphism of angiotensin converting enzyme(ACE) gene and cigarette smoking with ischemic stroke (IS) . Methods Four hundred and fifty - four patients and 334 controls were recruited in this investigation, their ID polymorphisms of ACE gene were detected by polymerase chain reaction (PCR) and denaturing high performance liquid chromatogram(DHPLC) . The correlation of genotypes and cigarette smoking and ischemic stroke risk were measured by odds ratios (ORs) and 95% confidence intervals (95% Cls ) . The ORs and 95% Cls were calculated by unconditional logistic regression. Results Compared to controls, OR of DD genotype and D allele in ischemic stroke patients of cigarette smoking was 1.992 and 1. 679, respectively; but that the OR of II genotype in ischemic stroke patients of sigarette smoking was 0.523. On the other hand, the OR of all genotypes and alleles in ischemic stroke patients of non - cigarette smoking were not significantly different than those of the controls. Conclusion Cigarette smokers with D alleles are likely to suffer from ischemic stroke, but those cigarette smokers with I(Ⅱ ?)alleles are not likely to suffer from ischemic stroke, and there is interactions between cigarette smoking and ACE gene in the pathogenesis of ischemic stroke.
