宫颈分泌物中胎儿纤维连接蛋白和血清白细胞介素-6在早产预测中的价值

目的了解先兆早产孕妇、早产孕妇和正常妊娠孕妇宫颈分泌物中胎儿纤维连接蛋白(FFN)、血清白细胞介素-6(IL-6)的水平,观察联合检测FFN、IL-6是否提高预测早产的准确性。方法使用ELISA法测定妊娠28~34周并诊断为先兆早产的孕妇41例(Ⅰ组)、早产的同孕龄孕妇40例(Ⅱ组)及相同孕周的正常孕妇60例(Ⅲ组)的宫颈分泌物FFN和血清IL-6含量,并追踪其妊娠结局。结果①Ⅰ,Ⅱ组的FFN均明显高于Ⅲ组(P均<0.05);Ⅱ组IL-6的含量高于Ⅲ组(P<0.05),Ⅰ组与Ⅲ组比较无显著性差异(P>0.05)。②FFN预测早产和1周内分娩的敏感度和阳性预测值高于IL-6,而特异度和阴性预测值则低于后者。结论①FFN和IL-6的浓度与早产相关。②FFN和IL-6均是预测早产和1周内分娩较为敏感的指标,联合检测2项指标将提高预测早产和2周内分娩的预测能力。

Value of cervical secretions fetal fibronectin and serum interleukin - 6 in predicting preterm delivery

Objective It is to understand the levels of cervical secretions fetal fibronectin(FFN) and serum interleukin-6(IL-6) in threatened premature labor gravida,preterm labor gravida and normal pregnancy gravida and to observe FFN and IL-6 combined detection can raise the accuracy of predicting preterm delivery or not.Methods The contents of cervical secretions FFN and serum IL-6 in 41 cases of threatened premature labor gravida whose pregnancy time was 28-34 weeks(group Ⅰ),40 cases of the same pregnancy time preterm labor gravida(group Ⅱ) and 60 cases of the same pregnancy time normal pregnancy gravida(group Ⅲ) were detected with ELISA method.The pregnancy outcomes of the three groups were tracked.Results ①The contents of FFN in group Ⅰ and group Ⅱ were both obviously higher than that in group Ⅲ(P<0.05).The contents of IL-6 in group Ⅱ were higher than that in group Ⅲ(P<0.05),but there was no significant difference between group I and group Ⅲ(P>0.05).②The sensitivity and the positive predictive value of FFN predicting preterm delivery and delivery within one week were higher than that of IL-6,but the specificity and the negative predictive value of FFN were lower than that of IL-6.Conclusion ①The concentrations of FFN and IL-6 are concerned with preterm delivery.②Both FFN and IL-6 are sensitive indexes for predicting preterm delivery and delivery within one week.The two indexes combined detection can raise the predicting ability of predicting preterm delivery and delivery within one week.
