胰岛素泵与门冬胰岛素30治疗初诊2型糖尿病的疗效比较

目的探讨胰岛素泵强化疗法与胰岛素皮下注射强化疗法治疗初诊2型糖尿病的临床疗效。方法将确诊为2型糖尿病的68例初诊患者随机分为两组,分别为对照组(胰岛素皮下注射强化治疗组)34例和观察组(胰岛素泵强化治疗组)34例。观察两组患者治疗前、后2周血糖及达标时间、用药量、胰岛素分泌指数(HOMA-BE)、胰岛素抵抗指数(HOMA-IR)。结果两组患者治疗后的空腹血糖、餐后2 h血糖水平均较治疗前明显下降;治疗前后观察组与对照组空腹血糖、餐后2 h血糖水平差异均无统计学意义(P>0.05)。观察组胰岛素用量、血糖达标时间、低血糖次数明显低于对照组(P<0.05或P<0.01);两组患者治疗后HOMA-IR均较治疗前明显降低。结论胰岛素泵强化治疗初诊2型糖尿病临床疗效与门冬胰岛素30三针皮下注射治疗疗效相当,但胰岛素泵强化治疗可缩短血糖达标时间,减少胰岛素用量,降低低血糖发生率。

Comparison of insulin pump and insulin aspart 30 in the treatment of newly diagnosed type 2 diabetes

Objective To explore the clinical efficacy of insulin pump intensive therapy and multiple subcutaneous insulin（aspart 30） injection in newly diagnosed type 2 diabetes.Methods 68 patients and newly diagnosed as type 2 diabetes were randomly divided into two groups,namely,the control group（subcutaneous insulin intensive therapy） with 34 cases and observation group（insulin pump therapy） with 34 cases.Results After two weeks treatment,fasting plasma glucose and 2 h postprandial blood glucose levels decreased markedly in both groups without significant differences between observation group and control group,but the dose of insulin,blood glucose control,the frequency of hypoglycemia in observation group,was significantly lower than those of the control group（P 0.05 or P 0.01）.HOMA-IR in both groups significantly reduced after the treatment.Conclusion The clinical efficacy of insulin aspart 30 three-pin subcutaneous injection is comparable with insulin pump in newly diagnosed type 2 diabetics.
