系统性红斑狼疮的超声心动图改变

本文收集我院确诊为系统性红斑狼疮(SLE)的51例病人的资料,并与20例年龄、性别和身高体重配对的正常人进行对比分析。超声检出40例异常,其中办膜返流19例、心包积液7例、左室扩大8例、左房扩大5例、异常室壁增厚1例。在所查9项超声指标中,以EPSS、LVd、LVs、E、A和E/A最敏感(P<0.05)。本文认为:1.超声能显示SLE患者的明显心包、心内膜及心肌损害;2.左心舒张功能损害先于收缩功能损害:3.超声能显示亚临床型SLE患者的异常。

Echocardiographic Abnormolities in Patients with Systemic Lupus Erythematosus

The purpose of the study was to evaluate the diagosis of the systemic lupus erythemato-sus (SLE) by noinvasive two-dimensional (2 - D), M-mode and Doppler Echocardiography.Methods A total consecutive SLE patients and 20 healthy sex-and age-mathed control subjects were evaluated. There were respectively 49 femal, two and 19 femal, One male in the patient groups and control group. There mean ages respectively 28. 98+9. 22 and 32. 8+7. 9 yuears. 51 cases of SLE patients were divided into three groups: mild. middle and severe groups. according to clinal condition.Results 40 cases of echocardiographic abnormalities were showed, including regurgitations of all kinds (19 cases, 47.5%), pericardial effusion (7 cases, 17.5%). left ventricular enlargement (8 cases, 20%), left atrium enlargement (5 cases, 12. 5%), one case of abnormal thickening of endocardium (2. 5%).Nine indexes of echocardiography were evaluated. Six of them, including EPSS, LVd, LVs, E,A, and E/A were significantly different (P<0. 05) betwent the control group and three groups of the patients.Conclusion 1. Obvious injures of the pericardium, myocardium and endocardium resulted from SLE can be showed by 2D M -made and Doppler echocardiography. 2. Left ventricular diastolic dysfunctions appear earlier then the left ventricular systolic dysfunction in the patients with SLE. 3. E-chocardiography is able to demonstrate subclinical myocardial involement.
