老年人不典型急性心肌梗死的临床特征分析及治疗体会

目的：分析老年人不典型急性心肌梗死的临床特征及治疗体会。方法：选取2005年6月～2009年6月本院收治的确诊为老年人不典型急性心肌梗死病例78例,回顾性研究其临床资料,分析老年人不典型急性心肌梗的误诊情况、临床特点、治疗方式及抢救成功率。结果：本组患者初次就诊时误诊26例（33.3%）,疑诊者24例（30.8%）,就诊时确诊者28例（35.9%）,平均住院时间为2～3（2.3±0.7）周,70例（89.7%）患者顺利出院,急诊室死亡6例（7.7%）,后期死亡2例（2.6%）。结论：老年人不典型急性心肌梗死首发症状变异明显,提高警惕才能防止诊断鉴别的局限性、片面性,提高诊断率,降低误诊发生的可能,为及时抢救创造条件。

Clinical features and treatment experiences of typical acute myocardial infarction in old people

Objective：To analyze the clinical features and treatment experiences of typical acute myocardial infarction in old people.Methods：From June 2005 to June 2009,78 cases of typical acute myocardial infarction in old people were selected in our hospital,the clinical data,clinical characteristics and treatment of them were retrospectively studied.Results：26 cases （33.3%） were misdiagnosed when the first medical diagnosis performed in the group,24 cases （30.8%） were sus pected,28 cases （35.9%） were diagnosed when the first medical diagnosis performed in the group.The average length of hospitalization was 2～3 （2.3 ± 0.7） weeks,70 patients smoothly discharged,6 cases（7.7%） died.Conclusion：Significantly changes can be observed from typical acute myocardial infarction in old people,alerting should be used to prevent the lim itations and one-sidedness of differential diagnosis.
