改良混合脱钙液在骨髓活检组织中的应用

目的 探讨改良混合脱钙液在临床骨髓组织活检中的应用。方法 选取临床骨髓穿刺活检病例188例,分别采用改良混合脱钙液（改良混合脱钙液组,146例）与传统硝酸类脱钙液（硝酸类脱钙液组,42例）在常温下进行脱钙处理,比较两组脱钙的时间、制成切片的质量、HE染色效果及免疫组化表型表达情况。结果 改良混合脱钙液的脱钙时间短于硝酸类脱钙液,改良混合脱钙液的切片质量良好为97.9%（143/146）,HE染色效果良好为98.6%（144/146）;硝酸类脱钙液切片质量良好为61.9%（26/42）,HE染色效果良好为54.8%（23/42）。改良混合脱钙液组标本免疫组化表达效果优于硝酸类脱钙液组。结论 改良混合脱钙液效果良好、脱钙时间短、染色效果佳、组织抗原不丢失、实用性强,能很好地满足临床骨髓组织活检的脱钙需要。

The Application of the Improved Hybrid Decalcify Fluid in the Bone Marrow Biopsy

Objective To discuss the application of the improved hybrid decalcified fluid in the bone marrow biopsy.Methods 188 bone marrow biopsy cases were selected and divided into two groups（A and B）.Improved hybrid decalcify fluid and 5% nitric acid decalcify fluid were used into A group with 146 cases and B group with 42 cases at the normal temperature respectively.Then the time of decalcification、the quality of slices、the effect of HE dyeing and the expression condition of the immunohistochemical phenotype were compared and analyzed.ResultsThe improved hybrid decalcified fluid was faster than nitric acid decalcify fluid in bone marrow decalcification.The rate of qualified slices in group A was 143/146（97.9%）,while that was 26/42（61.9%）in group B.The rate of qualified HE dyeing in group A was 144/146（98.6%）,while that was 23/42（54.8%）in group B.The expression of immunohistochemical phenotype in group A was better than that in the B group.Conclusion Through the comparison of the two decalcify fluids,the improved hybrid decalcify fluid has a lot of advantages,like better qualified slices、qualified HE dyeing、detection rate of immunohistochemical phenotype and less time in the bone marrow decalcify.Therefore,it can satisfy the need of decalcification in the bone marrow biopsy well.
