谷氨酰胺联合重组人生长激素对严重烧伤患者蛋白代谢和免疫功能的影响

摘    要:目的探讨联合应用谷氨酰胺(Gln)和重组人生长激素(rhGH)对严重烧伤患者蛋白代谢、免疫功能及创面愈合能力的影响。方法将60例严重烧伤患者随机分为对照组、rhGH组及Gln+rhGH组,每组20例。三组病人全部进行常规治疗外,rhGH组及Gin+rhGH组于伤后5天皮下注射rhGH(0.2—0.25U/kg/d),另外Gin+rhGH组静脉给予Gin(0.2g·kg-1·d-1)。于用药后7d、14d分析比较三组患者白蛋白、免疫球蛋白水平,并记录创面愈合时间及住院日。结果Gln+rhGH组血浆白蛋白及免疫球蛋白水平均高于rhGH组及对照组;30d创面愈合率高于rhGH组及对照组;住院日明显少于rhGH组及对照组。结论联合应用Gin和rhGH能有效促进蛋白合成,提供机体免疫功能,促进创面愈合,缩短住院日。

Abstract:Objective To study the effect of combined application of gintamine (Gin) and recombinant human growth hormone (rhGH) on the protein metabolism,immune function and wound healing capacity in severely burned patients. Methods 60 patients with severe burns were randomly divided into control group, rhGH group and Gin + rhGH group, with 20 patients in each group. All three groups of patients for routine medical treatment, rhGH group and Gin + rhGH group 5 days after injury, subcutaneous injection of rhGH ( 0.2 - 0.25 U/kg/d) , another Gin + rhGH group of intravenous Gin (0.2 g . kg-1 . d-1 ). Albumin, immunoglobulin levels, record wound healing time and the time of hospital stay were analyzed and compared between two groups on the 7th, 14th day after treatment analysis. Results Serum albumin and immunoglobulin levels were higher, wound healing rate was higher, the time of hospital stay was less in Gin + rhGH group than those in rhGH group and control group. Conclusion Gin combined with rhGH can effectively promot protein synthesis, impove immune function and promote wound healing, shorten hospital stay time.
