乙型肝炎病毒载量与血清学标志及preS1抗原的关系

目的探讨乙型肝炎病毒(HBV)载量与血清学标志及preS1抗原的关系,为乙型肝炎的诊断、疗效评估提供科学依据。方法采用荧光定量聚合酶链反应(FQ-PCR)和酶联免疫吸附试验(ELISA),检测患者血清HBVDNA载量、血清学标志及preS1抗原。结果155例HBVDNA载量大于103拷贝/ml的标本中,preS1阳性136例,HBeAg阳性100例,两种检测指标间存在显著性差异(P<0·01)。preS1/HBeAg组合分析检测HBV复制的灵敏度100%、特异性78·1%。preS1或HBeAg阳性患者HBVDNA拷贝数显著高于preS1或HBeAg阴性患者(P<0·01)。结论血清preS1、HBeAg与HBV复制密切相关。作为反映HBV复制的指标,preS1优于HBeAg,preS1/HBeAg组合分析优于preS1,preS1/HBeAg在HBV的诊断和疗效评估方面有重要的临床应用价值。

Relationship between the quantitation of HBV DNA and HBV markers or preS1 antigen

Objective To study the relationship between the quantitation of HBV DNA and HBV markers or preS1 antigen, and providing a scientific basis for diagnosis and evaluation of therapy of HBV. Methods Fluorescent quantitative PCR and ELISA were used to detect serum HBV DNA levels and HBV markers and preS1 antigen, respectively. Results In 155 patients whose HBV DNA levels were more than 10~3copies/ml, 136 patients were preS1 positive and 100 patients were HBeAg positive. There were a significantly difference between preS1 and HBeAg for detection of HBV replication(P<0.01). The sensitivity and specificity of preS1/ HBeAg for detection of HBV replication were 100% and 78.1%, respectively. HBV DNA levels in preS1 or HBeAg positive patients were significantly higher than preS1 or HBeAg negative patients(P<0.01). Conclusion Serum preS1 and HBeAg closely correlates with HBV replication. preS1 is better than HBeAg, and preS1/HBeAg is better than preS1 for detection of HBV replication.
