彩色多普勒超声对2型糖尿病下肢动脉病变的诊断价值

目的探讨彩色多普勒超声对2型糖尿病下肢动脉病变的诊断价值。方法应用彩色多普勒超声诊断仪观察82例糖尿病患者及30例健康对照者双下肢动脉内径、内-中膜厚度、血流动力学改变、斑块形成情况及有无管腔狭窄或闭塞。结果（1）双下肢动脉硬化发生率糖尿病组明显高于对照组（P〈0．05）。其中无症状组动脉硬化发生率65．3％。（2）糖尿病组下肢血管粥样硬化斑块、狭窄、闭塞的发生率明显高于对照组，动脉硬化斑块好发于股动脉，狭窄和闭塞好发于胫动脉及足背动脉。结论糖尿病患者动脉硬化发生率高，发病早，彩色多普勒超声对诊断2型糖尿病下肢动脉病变、预防下肢坏疽的发生具有重要临床价值。

The Diagnostic Value of Colorful Doppler Ultrasound for Lower Extremity Pathological Changes in Diabetes Patients

Objective To study the diagnostic value of colorful Doppler ultrasound for lower extremity pathological changes in diabetes patients. Methods Observe the inner diameter, thickness of inner-middle membrane, dynamic changes of blood, formation of plaque（clot） and narrowing or occlusion of bilateral lower extremity in 82 diabetes patients and 30 contrast health group by colorful Doppler ultrasound. Results （1）The morbidity of arterosclerosis in bilateral lower extremity in diabetes is significantly higher than the contrast health group. The morbidity of arteresclerosis in asymptomatic group is 65.3 %. （2）The morbidity of sclerotic plaque, narrowing and occlusion of lower extremity in diabetes patients is significant higher than the contrast health group. The sclerotic plaque is more often found in femoral artery, as well narrowing and occlusion in tibial artery and dorsal artery of foot. Conclusion High morbidity and early involvement of arterosclerosis are found in diabetes patients. It is of clinical importance to make diagnosis of pathological changes in lower extremity arteries in diabetes patients by colorful Doppler ultrasound and prevent the incidence of gangrene.
