超声心动图评价Ebstein畸形术前和术后心脏及瓣膜功能的研究

目的 :超声心动图评价 Ebstain’ s畸形术前、术后心脏及瓣膜功能。方法 :我们对 1987年～ 1999年超声心动图检查经手术证实的 3 9例三尖瓣下移畸形患者进行了回顾性研究。成人 3 1例 （年龄 19～ 4 7岁 ） ;儿童 8例 （年龄 7～15岁 ）。所有病例均并发房间隔缺损或室间隔缺损。结果 :彩色多普勒血流成象显示术前三尖瓣返流 （ TR） 2～ 3级 ,术后 TR明显减轻至 1～ 2级 ;60 %患者的功能右室与房化右室的比例得到改善 ,多数患者右室功能明显改善。结论 :右心功能和房化右室腔的大小代表该病的严重程度 ,彩色多普勒超声心动图可显示心脏形态并提供更多的血流动力学信息 ,它可作为判定 Ebstain’ s畸形手术成功的标准。

A study of cardiac and valvular function in Pre and post operation of Ebstein's anomaly assessed by echocardiography

AIM: To assess cardiac and valvular function in pre and post operation of Ebstein's anomaly by echocardiography. METHODS: In thirty nine patients, 31 adults (19~47 years) and 8 children (7~15 years), operated between 1987 and 1999 echocardiography was performed pre and post repair and at follow up. All patients had additional congenital cardiac defects (ASD, VSD). RESULTS: Color Doppler flow imaging revealed a severe preoperative tricuspid regurgitation (TR, grade 2 to 3). Postoperatively TR was significantly reduced to grade 1 to 2.60% of the patients demonstrated an improvement in the ratio of atrialized chamber to functional right ventricle, and right ventricular function was improved in most patients. CONCLUSION: The severity of the disease is represented by the size and function of the right ventricle and the atrialized chamber, color Doppler echocardiography should display the morphology and also provide some hemodynamic information, echocardiographically we were able to identify significant characteristics for the successful repair of Ebstein's anomaly.
