抗菌药物合理使用信息管理系统在抗菌药物管理中的实践

摘    要:
目的:提高医院抗菌药物的药事管理工作成效。方法:以数据库建设为核心内容,对临床信息系统(CIS)中医院感染相关信息进行提取、整合,设计由基础数据库建设、围手术期抗菌药物预防性应用监控等5个模块构成的抗菌药物合理使用信息管理系统,对医院整体用药情况进行电子监控。以我院骨外科为例,分别对2011年1-6月(干预前)和2012年1-6月(干预后)抗菌药物使用情况进行分析,并比较干预前、后医院抗菌药物使用强度、住院患者抗菌药物使用百分率等指标水平的变化情况。结果:初步建立了一个科学合理、操作简便的医院抗菌药物合理使用信息管理系统。干预前使用强度为66.94DDDs/100人/天,住院患者抗菌药物使用百分率为62.18%,干预后使用强度下降了13DDDs/100人/天(仅为53.22DDDs/100人/天),使用率下降7%(仅为55.7%)。结论:抗菌药物的信息管理能够及时、有效地规范医院抗菌药物应用,使之日趋合理,有效提高药事工作效率与管控效果。

Abstract:
OBJECTIVE: To improve the efficiency of pharmaceutical management of antibacterial drugs in hospital. METH- ODS: Taking the construction of database as core content, the relevant information of clinical information system was extracted and integrated. Information management system about rational use of antibacterial drugs was designed, involving the construction of basic database, perioperative prophylactic application of antibacterial drugs and other 3 modules. The real-time monitoring was carried on to the clinical use of antibacterial drugs in the hospital. Taking orthopedics surgery department of our hospital as example, the use of antibacterial drugs during Jan.-Jun. 2011 (before intervention) and Jan.-Jun. 2012 (after intervention) were analyzed, antibacterial use density (AUD), the utilization ratio of antibacterial drugs and other index were compared before and after intervention. RESULTS: Scientific, reasonable and convenient information management system about rational use of antibacterial drugs had been established initially. The AUD of antibacterial drugs was 66.94 DDDs/100 persons/day before intervention, and utilization ratio of antibacterial drugs was 62.18% in the inpatient. After intervention, two indexes were decreased by 13 DDDs/100 persons/day (to 53.22 DDDs/100 persons/day) and 7% (to 55.7% ). CONCLUSIONS: The information management of antibacterial drugs has tended to be timely, rational and standardized, which has greatly improved work efficiency and control effect of the pharmaceutical administration.
