外用癌痛消联合肿瘤深部热疗治疗急症骨转移癌爆发痛的临床研究

目的 观察外用癌痛消联合肿瘤深部热疗治疗急症骨转移癌爆发痛的临床疗效.方法 将96例急症骨转移癌症患者根据患者治疗急症骨转移癌爆发痛方式的不同采用随机、对照、双盲的方法分为对照组、安慰剂组与治疗组,各32例,对照组予癌痛消外敷治疗;安慰剂组予肿瘤深部热疗及安慰剂疼痛贴;治疗组予肿瘤深部热疗及外用癌痛消.观察3组患者治疗前后功能状态评分（KPS）、疼痛强度评分（NRS）、爆发痛次数;比较3组患者镇痛效果有效率的比较;3组患者常见并发症的发病率的比较.结果 3组患者治疗前的KPS、NRS、爆发痛次数差异无统计学意义（P＞0.05）,3组患者治疗后的KPS、NRS、爆发痛次数差异均有统计学意义（P＜0.05）.3组患者治疗急症骨转移癌爆发痛的镇痛效果的有效率、常见并发症的发病率差异均有统计学意义（P＜0.05）.结论 采用外用癌痛消联合肿瘤深部热疗治疗急症骨转移癌爆发痛,可以有效提高治疗有效率,减少在治疗过程中并发症的发病率,有效提高患者的生存质量.

Clinical Studies of Topical Treatment of Breakthrough Pain of Acute Bone Metastases with Pain Elimination Combined with Hyperthermia

Objective：To investigate the clinical effects of topical treatment of breakthrough pain of acute bone metastases with pain elimination combined with hyperthermia.Methods：96 cases of cancer patients were randomly divided into 3 groups-control group,the placebo group and treatment group-32 cases in each group,according to the different ways pain erupted with the randomized controlled method and double blinded method.The control group were treated with external application of Aitongxiao ;the placebo group were treated with plus tumor hyperthermia and placebo pain paste ; the treatment group were treated with external application of Aitongxiao and plus tumor hyperthermia.Before and after treatment,KPS score,NRS score and the times of breakthrough pain of the 3 groups were observed ; the efficiency of the analgesic effect and the incidence rate of common complications in the 3 groups were compared.Results：before the treatment,there was no obvious difference in KPS score,NRS score and the times of breakthrough pain of the 3 groups;after the treatment,there was significant difference in KPS score,NRS score and the times of breakthrough pain of the 3 groups（P＜ 0.05）;there was a significant difference（P＜ 0.05） in the analgesic effect of effective rates of three groups ;incidence rates of the 3 groups had significant differences （P ＜ 0.05）.Conclusion：In summary,the topical treatment of breakthrough pain of acute bone metastases with pain elimination combined with hyperthermia can effectively improve the efficiency of the therapeutic treatment of breakthrough pain of acute bone metastases,reduce the incidence of complications in the healing process,and effectively improve the quality of life of patients with cancer pain,worth promoting for further study.
