腹腔镜对盆腔脓肿的诊治价值

目的:探讨腹腔镜手术在诊断和治疗盆腔脓肿中的应用价值。方法:回顾分析本院1999年2月～2004年12月35例盆腔脓肿腹腔镜诊断和处理的临床资料。结果:34例在腹腔镜下完成手术,1例因盆腔严重粘连而中转开腹,无1例并发症。术前、术后诊断符合率72.9%(22/35)。手术时间40～80min,平均51min。术后住院5～7天,平均5.1天。术后病率7例,4天均恢复正常。术后3天血常规全部恢复正常。所有病例随访6个月,无1例复发。17例要求生育的患者于术后1～3个月行输卵管通畅检查术,13例通畅,7例已妊娠。结论:早期的腹腔镜手术有利于提高盆腔脓肿的确诊率,有利于保护患者生育功能,对于未育患者尤为适用。

Application of laparoscopic operations for pelvic abscess

Objective:To investigate the value of laparoscopic operations for the diagnosis and treatment of pelvic abscess.Methods:A series of 35 patients with pelvic abscess diagnosed and treated by laparoscopic technique from Feburary 1999 to December 2004 were retrospectively analyzed.Results:Open surgery was required in 1 patient because of serious pelvic adhesions.No complications were observed.The diagnostic accordance rate before and after the laparoscopy was 72.9%(22/35),the duration of operation was 40~80 min(mean,51 min)and the postoperative hospital stay was 5~7 d(mean,5.1 d),postoperative pyrexia occurred in 7 patients,whose temperatures retuned to normal with 4 d.The blood routine examination results were restored to normal levels in all patient at 3 d after operation.No relapse occurred follow-up checkup for 6 months in all the patients.Fallopian tubes of 13 patients were unobstructed among 17 patients requesting for pregnancy,under fallopian tubes unobstructed at post-operative 1~3 months.7 patients had gestated until now.Conclusion:Early laparoscopic operations is benefit for higher diagnostic rate of pelvic abscess and for protection of procreative function,especially for un-procreation patients.
