摘 要: | 目的:调查妇女生殖道高危型人乳头瘤病毒(Human papillomavirus,HPV)感染状况,研究高危型HPV感染与宫颈癌及癌前病变的关系。方法:应用第二代杂交捕获(hybrid capture,HC-Ⅱ)实验对30~49岁的已婚妇女进行高危型HPV DNA检测,采用多因素的非条件Logistic回归模型分析HPV感染与宫颈癌及宫颈上皮内病变(CIN)的关系。结果:调查人群高危型HPV DNA总检出率为18.7%,与正常组相比检出率随病变程度加重呈趋势性增高(χ2=444.04,P<0.001)。各年龄组宫颈HPV DNA的检出率几乎一样,无显著性差异(χ2=10.25,P>0.001)。非条件Logistic回归分析显示,HPV感染与宫颈上皮内高度病变(CINⅡ)及癌症的发生高度相关(OR=121.1)。结论:生殖道高危型HPV感染是当地宫颈癌及宫颈上皮内瘤变流行的主要危险因素,提示宫颈癌的防治重点应放在防止HPV感染、对HPV感染的筛查和密切监测已感染高危型HPV的对象。

Abstract: | Objective:To investigate the infective situation of high risk human papillomavirus(HPV) in women,research the relationship between high risk HPV infection and cervical cancer,precancerous lesion.Methods:Hybrid capture-Ⅱ(HC-Ⅱ) method was used to detect high risk HPV DNA among married women aged 30~49 years old,multivariate non-conditional Logistic regression analysis was used to analyze the relationship between high risk HPV infection and cervical cancer,cervical intraepithelial neoplasia(CIN).Results:The total detection rate of high risk HPV DNA was 18.7%,increasing with aggravation of the disease compared with normal group(χ2=444.04,P<0.001);there was no significant difference among different age groups(χ2=10.25,P>0.001).Multivariate non-conditional Logistic regression analysis indicated that there was a correlation between HPV infection and cervical cancer,CIN(OR=121.1).Conclusion:High-risk HPV infection of female genital tract is a major risk factor of cervical cancer and CIN in Xiushui region,which indicates that the key point of prevention and treatment of cervical cancer should be focused on avoiding HPV infection,screening women for HPV infection and monitoring population infected by high-risk HPV. |
