甘肃省会宁县高血压病综合干预效果评价

目的 以甘肃省会宁县高血压病人群综合干预研究为基础,探讨中国欠发达地区高血压防治的可行性及效果.方法 采用整群研究方法,以甘肃省会宁县基线调查发现的高血压患者为研究对象,将研究对象分为一般人群、高危人群和高血压人群3部分进行综合干预.高血压患者又分为1级高血压（轻度）、2级高血压（中度）和3级高血压（重度）.对一般人群和高危人群以健康教育和改变不良生活行为方式为主,对高血压人群按分级管理原则进行定期随访,采用药物和非药物相结合的综合措施,干预随访3年.结果 高血压患病率（15岁以上人群,下同）由19.37％下降到17.96％,人群高血压防治知识知晓率由30.76％上升到89.44％,治疗率由28.54％上升到45.74％,随访率由26.76％上升到97.62％,体检率由27.42％上升到96.34％,建档率由51.23％上升到99.80％,规范管理率由26.66％上升到71.52％,不良生活行为改变率由31.28％上升到42.39％,体重指数达标率由35.44％上升到46.49％,血压控制率由11.98％上升到46.81％.结论 甘肃省会宁县高血压综合管理采取全人群综合干预策略效果显著,此方法可作为经济欠发达地区全人群高血压防治和管理模式.

Comprehensive Intervention of Hypertension Management in Huining County of Gansu Province

OBJECTIVE to study the feasibility and effectiveness of comprehensive intervention of hypertension crowd. METHODS method takes a whole flock to the baseline survey and physical examination found all of the integrated interventions in hypertensive patients as research subjects. People crowd into the general population, high-risk and high blood pressure. Hypertension was divided into： I grade hypertension（mild）, secondary hypertension（moderate）, III stage hypertension（severe）. Health education to the general population and high-risk groups and changing bad living behavior, principles of classified management of hypertension, regular follow-up, the use of drug and non-drug combination of comprehensive measures. RESULTS the prevalence of hypertension（referring to people over the age of 15, the same below） dropped from 19.37% to 17.96%; hypertension prevention and control knowledge rate in- creased from 30.76% to 89.44%; treatment increased from 28.54% to 45.74%; follow-up rates increased from 26.76% to 97.62%; medical examination rate increased from 27.42% to 96.34%; filing rate increased from 51.23% to 99.8%; standardized rates from 26.66% Up to 71.52%; bad behaviour change rate increased from 31.28% to 42.39%; body mass index standard rate increased from 35.44% to 46.49%; blood pressure control rate increased from 11.98% to 46.81%. CONCLUSIONS adopted comprehensive population-wide intervention strategies, or can be used as a crowd control measures and management models.
