急诊监护病房肺部感染患者的细菌学调查

摘    要:
目的:分析急诊ICU肺部感染患者细菌学特点及其耐药性,以制定合理的防治措施。方法:回顾分析急诊ICU肺部感染患者的临床特征、痰细菌学特点及其耐药性。结果:急诊ICU患者肺部细菌感染的发生率和病死率分别为15.87%和38.37%;86例患者中单菌株感染38例(44.19%),两株以上混合感染19例(22.1%),细菌和真菌混合感染29例(33.72%);共分离病原菌264株,其中G-菌132株(50.00%)、G 菌87株(32.95%)、真菌45株(17.05%);G-菌以鲍曼不动杆菌最多见(23.48%),G 菌以金黄色葡萄球菌最多见(10.23%);药敏显示各菌耐药水平较高。结论:急诊ICU危重患者肺部以耐药菌混合感染多见,应加强病原学监测以指导抗生素治疗。

Abstract:
Objective To prevent pulmonary infection in emergency intensive care unit.Methods The clinical characteristics, the sputum bacteriology and the antibiotics resistance were retrospectively analyzed in the patients with pulmonary infection in our intensive care unit.Results The morbidity and mortality of pulmonary bacteria infection were 15.87% and 38.37% respectively.Among the 86 cases of pulmonary bacteria infection,there were 38 cases of single bacteria infection(44.19%),19 cases of more than two bacteria infection(22.1%),and 29 cases of bacteria and fungus infection(33.72%).There were 264 isolates which included 132 G-isolates,87 Gram isolates and 45 fungus isolates.Acinetobacter baumannii(23.48%) and staphylococcus aureus(10.23%) were the most common G-and G isolates bacteria respectively,which had a multiple-antibiotics resistance. Conclusion The most common pulmonary infection is the mixed infection with multiple antibiotic resistance pathogens.It is very important to monitor the pathogen so as to guide the clinical use of antibiotics.
