急性心肌梗死的院前溶栓治疗

目的观察急性心肌梗死(AMI)不同时间溶栓治疗的血管再通率。方法62例无溶栓禁忌症患者均给经鼻导管吸氧,肠溶阿司匹林300 mg嚼服,0.9%NS 20 ml 尿激酶50万U静脉注射,继以尿激酶100万U加入80 ml生理盐水经输液泵静脉输注溶栓治疗。计算发病30 min溶栓,30 min~2 h、2~4 h、4~6 h及6~8 h内溶栓再通率,并把4 h内溶栓病例与4~8 h溶栓病例进行统计学比较。结果4 h内溶栓再通率75%,4~8 h溶栓再通率46.6%,两者比较差异有统计学意义。结论AMI溶栓时间越早再通率越高,因此在安全的前提下尽早溶栓。

Thrombolytic Therapy for Patients with Acute Myocardial Infarction

Objective To observe the rate of recanalization in patients with acute myocardial infarction after thrombolytic therapy with urokinase at different time. Methods 62 patients treated with intravenous thrombolytic therapy with urokinase, inhale O2, and aspirin. Rates of recanalization in 1/2 - 2 h, 2 -4 h, 4 - 6 h, 6 - 8 h were ohserved. Rate of recanalization in 4 hours was compared the rate in 4 - 8 hours. Results Rate of recanalization in 4 houps was 75% , and it was 46.6% in 4 - 8 hours. Conclusions Earlier thrombolytic therapy can increase rate of recanalization.
