紫杉醇联合化疗方案治疗晚期胃癌的临床疗效

目的：观察国产紫杉醇（PTX,商品名为特素）联合国产草酸铂（OXA,商品名为艾恒）、亚叶酸钙（CF）、5-氟尿嘧啶（5-Fu）,治疗晚期胃癌的近期疗效及毒副反应。方法：选取病理检查证实的晚期胃癌患者24例,所有患者均有可评价病灶。治疗方案为：PTX60mg/m^2iv gtt d1,d8,d15;CF100mg/m^2,iv gtt d1-d5;5-Fu 375mg/m^2,iv gtt d1-d5;OXA65mg/m^2 iv gtt d1,d8。以上方案每28天为1周期,所有患者至少接受2周期治疗。结果：经2周期以上治疗后,CR2例,PR13例,NC5例,PD4例。总有效率62.5%,主要毒副反应为恶心、呕吐,骨髓抑制,脱发,外周神经感觉异常,肝肾功能轻度损害。结论：国产紫杉醇联合草酸铂,亚叶酸钙,5-氟尿嘧啶,治疗晚期胃癌疗效较好,毒副反应可耐受,值得临床推广应用。

Preliminary clinical observation of paclitaxel based chemotherapy in the treatment of advanced gastric cancer

Objective：To evaluate the efficiency and toxicity of paclitaxel based chemotherapy oxaliplatin, leucovorin and fluorouracil in the treatment of advanced gastric cancer. Methods. Twenty four measurable patients pathologically were enrolled into this study. The chemotherapy regimen was comprised of paclitaxel 60mg/m^2, iv gtt d1, dS, d15; oxaliplatin 65mg/m^2 ,iv gtt d1 ,d8; leucovorin 100mg/m^2 iv gtt d1 -d5; 5 -Fluorouracil 375mg/m^2 ,iv gtt/ d1 -d5. The regimen was given per 28 days. All enrolled patients received at least two cycles of treatment. Results： After two cycles of chemotherapy ,complete response was achieved in 2 cases ,partial response in 13 cases, stableness in 5 cases and progression in 4 cases. The overall response rate was 62.5%. The main side effects were nausea, vomiting, leucopenia and peripheral sensitive neutropathy. Conclusion： Paclitaxel bsaed chemotherapy has high release rate but comparasively mild toxicity for advanced gastric cancer patients.
