乌司他丁对肺癌围术期深静脉血栓形成的影响

目的分析乌司他丁对肺癌围术期深静脉血栓(deep venous thrombosis,DVT)形成的影响。方法选择进行肺癌切除术的患者174例,随机分成观察组和对照组,各87例。观察组手术前后应用乌司他丁,对照组手术前后应用低剂量肝素,比较两组患者手术前后的凝血功能和DVT形成情况。结果观察组术中出血量和术后引流量分别为(283.42±50.53)mL和(357.92±86.63)mL,对照组术中出血量和术后引流量分别为(309.26±59.81)mL和(560.81±120.69)mL,两组比较差异有统计学意义(P<0.05);两组患者手术后3 d的凝血功能和DVT发生率比较,差异无统计学意义(P>0.05)。结论乌司他丁和低剂量肝素均能有效改善患者围术期的高凝状态,但应用乌司他丁患者手术前后的出血量较低。

Effects of Ulinastatin on Deep Venous Thrombosis during Perioperative Period in Lung Cancer

Objective To analyze the effects of ulinastatin on deep venous thrombosis for lung cancer perioperative period. Methods A hundred and seventy- four cases of lung cancer resection in our hospital were selected and randomly divided into the observation group and the control group, 87 cases in each group. Ulinastatin was used in the observation group before and after operation, the low dose of heparin was used in the control group before and after operation. The blood coagulation function and deep venous thrombosis before and after operation were compared between the two groups. Results The amount of intraoperative bleeding and postoperative drainage volume were （283.42 ＋50.53）mL and （357.92 ~86.63）mL in the observation group,and （309.26 ：~59.81）mL and （560. 81 ~ 120. 69）mL in the control group,the difference between the two groups was statistically significant （P 〈 0.05 ）;the coagulation function and the inci- dence of DVT on postoperative 3 d in the two groups had no statistical significance （P 〉 0. 05）. Conclusion Ulinastatin and low dose of heparin can effectively improve the perioperative hypercoagulable state, but the bleeding amount before and after operation in the pa- tients with ulinastatin is lower.
