大脑后动脉假性动脉瘤的影像特点及介入治疗

摘    要:目的探讨大脑后动脉假性动脉瘤的影像特点和血管内介入治疗的方法及安全性。方法对8例大脑后动脉假性动脉瘤患者行影像学检查和介入检查。结果8例患者均行数字减影血管造影(DSA)检查,表现动脉瘤破损和对比剂外溢进入瘤腔,并显示假性动脉瘤大小形态;4例行MRI检查表现为囊性结构,T1与T2加权相上是混杂信号,可见流空现象。2例行动脉瘤瘤内完全致密填塞术,1例死亡,1例预后良好;6例行载瘤动脉闭塞术,其中1例出现暂时性视野偏盲,经治疗后恢复,余患者愈后良好。结论DSA检查对大脑后动脉假性动脉瘤的定性、定位最准确,介入治疗对假性动脉瘤的效果较好,但长期疗效有待于进一步观察。

Abstract:Objective To investigate the imaging character of pseudoaneurysm of posterior cerebral artery,the strategy and safety of intravascular therapy.Methods Eight cases with pseudoaneurysm of posterior cerebral artery were performed imaging examination and interventional therapy.Results Eight cases were examined by DSA,which showed the rupture of arteries,contrast material extravasations and formation of the cysts.Four cases examined by MRI showed cystic structures,low or high signals in T1W1 and T2W2,and the signal of blood flow can be seen.Two cases had complete thickening and plugging operation in tumor artery,one of which died and the other had better prognosis;six cases had occlusion operation in tumor artery,one of which had temporary blind visual field,who recovered after the treatment,the others were cured.Conclusion DSA is the best method to diagnose the pseudoaneurysms of posterior cerebral artery.Interventional treatment is the most important therapeutic method for pseudoaneurysm,but its long-term therapeutic effect needs further investigation.
