| 摘 要： | 应用心内膜心肌活检和电生理检查相结合的方法,研究新近发生的房室传导阻滞(AVB)12例患者的病因、阻滞部位及去自主神经影响药物实验的意义。12例中有半数诊断为心肌炎。3/4(9/12)的病例阻滞部位在房室结,希氏束内或室内束支者3例,有2例病因为非心肌炎。去自主神经影响药物试验阳性仅能反映有无迷走神经张力的影响,而不能确定同时有无器质性心脏病变存在。由心肌炎导致的AVB多具有动态变化特点。其它心脏疾患所致者治疗效果欠佳。

| Abstract: | The etiology and blocking level as well as the influence of autonomic blockade on atrioven- tricular block (AVB) occurring recently were evaluated by endomyocardial biopsy (EMB) and electrophysi- ological study (EPS) in 12 patients of recent A-V block. The pathohistological diagnosis in half of the cases was myocarditis. In three fourths of the patients, the blocking level was at atrioventricular (AV) node; and in another 3 cases the blocking level was in the hisbundle or bundle branch. The etiology of two among the three was not myocarditis. The positive result of autonomic blockade test could only reflect whether there was vagal influence on AV conduction, but it could not be used to rule out organic heart disease in the same cases. The AVB caused by myocarditis showed a characteristic changeable pattern and the treatment did not improve the AV conduction in the AVB caused by other heart disease. |
