肺真菌病CT诊断的探讨(附18例分析)

目的探讨肺真菌病CT表现特征，提高肺真菌病的诊断水平。方法回顾分析肺真菌病18例，痰培养9例、手术病理证实5例、纤支镜刷检及活检4例。全部病例均行肺部CT检查。结果左肺5例，右肺7例，双肺多发4例。病灶呈结节或肿块4例；肺亚段和肺小叶实变6例；厚壁空洞3例；霉菌球5例。结论CT对有特征性表现的肺真菌病可做出正确诊断，对于表现不典型者，密切结合临床可有效提供诊断准确率。

CT findings of pulmonary mycosis: report of 18 cases

Objective To study the CT findings of pulmonary mycosis and to improve its radiological diagnosis. Methods The CT findings of 18 cases of pulmonary mycosis confirmed by surgery, sputum culture, flexible fiberoptic bronchoscopy brush specimens or lung biopsy were retrospectively analyzed. CTscan was performed on all cases. Results There were 5 cases in the left lung, 7 in the right lung, and 4 cases in both lung. 4 cases showed nodule or masses appearance, 6 segmental or lobar consolidation within the lung filed, 3 cavities with thick wall and 5 fungal balls. Conclusion Pulmonary mycosis with characteristic manifestation can be correctly diagnosed by CT scan. Close combination with clinical findings can effectively improved the diagnosis accuracy in cases without typical manifestation.
