钛网不同塑形方法对颅骨缺损成形手术疗效的作用

摘    要:
目的比较术中手工塑形三维钛网、计算机辅助预塑钛网修补颅骨缺损的临床效果。方法2007年6月至2013年6月行颅骨修补56例患者的临床资料,其中使用术中手工塑形钛网35例和计算机辅助预塑钛网21例,分析比较两组患者疗效及并发症发生情况。结果手工塑形钛网组单侧颅骨缺损修补手术时间为(157.1±35.8)min比计算机辅助预塑钛网组(112.2±29.5)min明显延长(P0.05)。两组之间术后并发症如头皮下感染、修补材料外露、癫痫、皮下积液、皮肤慢性切割溃疡等发生率均无明显差异(P0.05)。而手工塑形钛网组术后塑形满意度低,计算机辅助预塑钛网组高,两组之间差异明显(P0.05)。结论预塑钛网修补颅骨缺损麻醉及手术时间短,塑形满意度高,患者生活质量较高。

Abstract:
Objective The effect of intraoperative hand-shaped and computer-aided shaped titanium mesh on cranioplasty is studied.Methods A total of 56 patients treated with cranioplasty from June 2007 to June 2013 were analyzed retrospectively,including 35 cases by intraoperative hand-shaping method and 21 cases by computer-aided shaping rethod.Results Unilateral repair time by hand-shaping method [(157.1 ± 35.8) min] was significantly longer than that by computer-aided shaping method [(112.2 ±29.5) min] (P 〈0.05).There was no significant difference between two groups in postoperative complications such as scalp infections,exposure of titanium mesh,epilepsy,subcutaneous effusion,and chronic skin ulcers (P 〉 0.05).The lower satisfaction rate by hand-shaping method was much higher than that by computer-aided shaping method (P 〈 0.05).Conclusion Computer-aided shaped titanium mesh in cranioplasty has the advantages of shorter surgical time,higher satisfaction rate and better quality of life for the patients.
