联合降压治疗对高血压病患者外周血中血管活性物质的影响及意义

目的观察非洛地平联合坎地沙坦对高血压患者血浆尾加压素Ⅱ（UⅡ）、脂联素（ADI）和血管紧张素Ⅱ（Ang Ⅱ）的影响。方法将88例入选患者作为治疗组,24例健康人群作为对照组,治疗组给予非洛地平及坎地沙坦治疗,测定治疗组患者药物治疗前后的外周血和对照组外周血中UⅡ、ADI、AngⅡ变化。结果治疗组UⅡ和AngⅡ水平高于对照组（t=3.961和t=2.816,均P〈0.01）,ADI低于对照组（t=16.243,P〈0.01）。治疗组治疗后较治疗前血压下降明显（P〈0.01）;UⅡ降低（t=2.024,P〈0.05）,ADI升高（t=5.589,P〈0.01）。结论非洛地平联合坎地沙坦可降低高血压患者外周血UⅡ水平,升高ADI、Ang Ⅱ水平。降低血压后,减轻了对ADI的抑制,减少了对UⅡ释放的诱导,提示联合降压治疗对高血压患者具有血管保护作用。

Significance and effect of combination antihypertensive therapy on vasoactive substances in peripheral blood in patients with hypertension

[Objective]To observe the effect of candesartan combined with felodipine on plasma urotensin Ⅱ（ UⅡ）,adiponectin（ ADI） and angiotensin（ Ang II） in patients with hypertension.[Methods]88 patients with hypertension were considered as the treatment group,and 24 healthy people were enrolled in the control group. The treatment group was given the candesartan and felodipine. The levels of UⅡ,ADI and AngⅡ in peripheral blood of two groups before and after treatment were measured.[Results]The levels of UⅡ and AngⅡ in the treatment group were higher than those in the control group（ t = 3. 961 and t = 2. 816,both P 0. 01）,while ADI level in the treatment group was lower than that in the control group（ t = 16. 243,P 0. 01）. In the treatment group,the blood pressure after treatment significantly decreased as compared with before treatment（ P 0. 01）,UⅡ level reduced（ t = 2. 024,P 0. 05）,and ADI level increased（ t = 5. 589,P 0. 01）.[Conclusion]Candesartan combined with felodipine can reduce UⅡ level,and increase levels of ADI and Ang II in peripheral blood in patients with hypertension. When blood pressure has been controlled,the inhibition of ADI is alleviated,and the induction of U Ⅱ release is reduced,and it is suggested that combination antihypertensive therapy has the protective effect on blood vessel of patients with hypertension.
