高血压患者血浆纤维蛋白原、D-二聚体和高敏C-反应蛋白水平分析及相关性研究

目的探讨各级原发性高血压(EH)患者血浆纤维蛋白原(Fib)、D-二聚体(DD)和高敏C-反应蛋白(hs-CRP)水平变化以及这三项指标之间的相关性。方法分别测定100例各级EH患者血浆Fib、DD和hs-CRP含量,以36例健康人作对照。血浆Fib测定采用比浊法,DD测定采用ELISA法,hs-CRP测定则采用乳胶增强免疫透射比浊法。结果血浆Fib水平在Ⅰ级EH时升高不明显(P>0.05),Ⅱ级和Ⅲ级时显著升高(P<0.05;P<0.01),Ⅱ级EH时的水平与Ⅰ级时比较差异不明显(P>0.05);DD水平在Ⅰ级EH时与对照组比较无统计学意义(P>0.05),其余各组间比较差异有显著性(P<0.01);hs-CRP水平各级EH患者间比较均P<0.01。各级EH患者血浆三项指标之间有良好的相关性(分别为γ=0.648,γ=0.831和γ=0.734;P<0.01)。结论各级EH患者血浆中hs-CRP水平变化比Fib和DD明显。血浆hs-CRP、Fib和DD用于联合检测,能较好地反映EH患者的病情发展程度,为其早期诊断和治疗提供依据。

Study of the relationship and the plasma level analyses of fibrinogen, D-dimer and high-sensitivity C reactive protein in hypertensive patients

Objective To investigate the level changes of the plasma fibrinogen(Fib),D-dimer(DD)and high-sensitivity Creactive protein(hs-CRP)in each grade essential hypertensive(EH)patients and the relationship among the three indicators.Methods plasma Fib,DD and hs-CRP content of 100 each grade EH patients were determined and compared with those of 36 healthy persons as control group.Plasma Fib was determined by turbidimetry,DD was determined by enzyme-linked immunosorbent assay,hs-CRP was determined by particle enmutarbidimetric method.Results Plasma Fib level was rised unapparently in Ⅰ grade EH(P>0.05),but it was all rised significantly in Ⅱ and Ⅲ grade EH(P<0.05;P<0.01),the level of Fib was unapparently different between Ⅱ grade EH and Ⅰ grade EH(P>0.05).The plasma DD level was no significant difference in Ⅰ grade EH compared with control group(P>0.05),there was significantly differenece among other groups(P<0.01).The hs-CRP level was significantly difference among all groups(P<0.01).There had well correlations were found among plasma Fib,DD and hs-CRP in all EH patients(γ=0.648,γ=0.831 and γ=0.734;P<0.01).Conclusion The plasma hs-CRP level was changed more apparently than Fib and DD in all EH patients.The results suggested that the combination measurement of plasma Fib,DD and hs-CRP might better evaluate the development process of EH patients,and be of help for early diagnosis and therapy of EH patients.
