直肠癌淋巴结转移相关影响因素的临床研究

目的：探讨直肠癌淋巴结转移的规律，为临床诊断及治疗提供参考。方法：对1992～2005年手术治疗的918例直肠癌的资料进行分类整理，分析其与淋巴结转移的关系。结果：肿瘤所在部位与淋巴结转移无明显关系（P〉0．05）。Logistic多因素回归分析得出相关因素与淋巴结转移的密切程度依次为：分化程度〉浸润深度〉侵犯肠管周径〉大体类型，分化程度是影响淋巴结转移的最主要因素。结论：直肠癌淋巴结转移与患者年龄、肿瘤大小、侵犯肠管周径、浸润深度、分化程度、大体类型、生长方式相关，其中肿瘤的分化程度是影响淋巴结转移的最主要因素。

Influencing factors relative to lymphatic metastasis of rectal carcinoma

Objective:To explore the nature of lymphatic metastasis of rectal carcinoma and benefit with clinical diagnosis and treatment.Methods:The data of 918 cases receiving operation between 1986 and 2002 were analyzed for the relationship with lymphatic metastasis.Results:Tumor site was not apparently related to the lymphatic metastasis(P>0.05).The degree of relevance was as follows,from more to less,by logistic regression analysis: the differentiating degree of the tumor,the affected depth,the diameter of affected bowel,the massive type of the tumor.The most major factor was the differentiating degree.Conclusion:The lymphatic metastasis of rectal carcinoma is related to age,tumor size,the diameter of affected bowel,the affected depth,the differentiating degree of the tumor,the massive type of the tumor and the growing pat-tern of the tumor.The differentiating degree is the major influencing factor.
