摘    要:目的:评估X线与CT对强直性脊柱炎患者骶髂关节炎的诊断价值。方法:对72例诊断为强直性脊柱炎的X线平片及15例CT片进行回顾性分析和总结。结果:单侧骶髂关节改变6例,双侧骶髂关节改变66例,表现为轻度骶髂关节炎30例,中度骶髂关节炎25例,重度骶髂关节炎17例;髋关节改变37例;72例HLA-27抗原阳性61例。结论:骨盆、骶髂关节正位片及左、右斜位片能对大部分骶髂关节炎作出诊断,尤其左、右斜位片更利于骶髂关节病变的观察,而CT扫描在显示细小病变方面更具有高敏感性。

Abstract:Objective To assess the usefulness of X-ray and CT for diagnosing and differentiating the sacroiliitis accompa- nying ankylosing spondylitis(AS).Methods X-ray and CT finding of 72 cases of AS were analyzed and reviewed retrospective- ly. Results The pathologic changes of unilateral and bilateral sacroiliac joints were found in 6 cases and 66 cases,respectively. Among all patients,mild sacroiliitis were found in 30 cases,mederate saeroiliitisin 25,and severe sacroiliitis in 17.The patho- logic changes of hip joints in 37 cases.The positive HLA-B27 antigen was found in 61 of 72 patients.Conclusions The an- teroposterior and oblique radiographs of the spine can correctly diagnose most saeroiliitis,especially left and fight oblique radio- graphs are useful to observe the lesions of the sacroiliac joint,and the CT scan in showing the small pathological changes has the high sensitivity.
