| 摘 要： | 
目的回顾上海市家庭医生签约服务历程，分析工作进展及存在的主要问题，提出建议与设想，为试点工作在全市推广与深化提供参考。方法依据上海市社区卫生服务综合改革云管理APP，采集并分析上海市各区社区卫生服务中心试点工作相关数据，并通过专家咨询法，对知情人进行访谈。结果上海市全部239家社区卫生服务中心均已启动了“1+1+1”签约服务试点，签约居民超过335万人，其中60岁及以上老年人签约率达到65.80%；为签约居民开具延伸处方超过91万张；签约居民门诊在“1+1+1”签约医疗机构组合内就诊为74.69%，在社区卫生服务中心就诊接近70%，在签约社区就诊达53.84%。结论签约试点进展良好，但仍然存在各社区试点进度不一、就诊频次过高、家庭医生签约服务积极性受限及供给模式单一等问题，需要从试点推广、落实服务举措、夯实运行机制、借助信息技术等方面进一步完善。
