摘    要:目的:通过对66例有典型颈椎病临床症状并且有完整CT、CR检查资料,临床怀疑颈椎病的患者进行回顾性分析,探讨CT和CR对颈椎病早期的诊断价值。方法:对66例患者均行CR检查和CT平扫检查,CR拍摄颈椎侧位及左右斜位共三个位置,CT平扫C2~C6椎间盘,每个椎间盘扫描6层。结果:CR检查66例中,未见异常30例,颈椎曲度异常诊断颈椎病早期36例(54.5%)。CT检查66例,共扫描330个椎间盘层面。其中CR检查未见异常组30例,CT最后诊断颈椎病21例(70%)。颈椎曲度异常组36例,诊断颈椎病36例(100%)。结论:CT能良好显示椎间盘的突出或膨出程度、椎管及横突孔的大小形态、锥体后缘增生及后纵韧带骨化等,对颈椎病早期的诊断有效、准确、可靠。颈椎曲度异常是CR诊断颈椎病早期的重要征象,但是诊断准确率远不及CT。

Abstract:Objective To investigate the diagnostic values of CT and CR in the early stage cervical spondylosis by analysing the CT and CR documents of 66 patients with clinically suspected cervical spondylosis retrospectively. Methods CR and CT studies were performed in 66 patients. Lateral , left and right oblique views were done by CR. Conventional CT scan of cervical discs was done from C2 to C6,6 slices for one disc. Results Thirty of 66 patients were normal and thirty-six (54.5%) with abnomalities of cervical curve were diagnosed as the early stage cervical spondylosis according to CR studies. Out of thirty patients which normal on CR, twenty one were diagnosed as cervical spondylosis by CT. And all of the thirty-six patients diagnosed as early stage cervical spondylosis by CR were also confirmed by CT. Conclusion CT is reliable in diagnosis of the early stage cervical spondylosis which is more useful and accurate than CR.
