老年高血压患者的社区药学服务体会

目的探讨老年高血压患者的社区药学服务方式。方法对已确诊的高血压患者进行定期随访和药学咨询服务,并进行医学干预,共随访290例。干预前后对居民高血压防治知识的认知率进行调查统计;干预前后老年高血压患者的服药依从率、血压控制率、血压知晓率评价和全年血压指标变化分析。结果干预后人群对高血压防治知识的认知率达到90%以上;干预后血压动态观察96%的患者血压降至正常水平。290例高血压患者经药学服务干预后,血压知晓率和控制率有显著提高（P〈0.01）。结论采用主动的药学服务方式,有助于提高社区老年高血压病的知晓率、治疗率、控制率,是控制高血压的有效途径。

Experience in Community Pharmaceutical Care of Old Patients with Hypertension

Objective To explore method of community pharmaceutical care of old patients with hypertension.Methods Confirmed patients with hypertension were regularly visited and received pharmaceutical care with medical intervention.Totally 290 patients were visited.Investigation with statistics on residents＇cognitive rate in preventing and treating hypertension before and after the intervention.Medication compliance rate,hypertension control rate,hypertension awareness rate assessment and annual blood pressure index change were analyzed.Results After medical intervention,the crowd＇s cognitive rate of preventing and treating hypertension reached over 90%;and after the intervention,blood pressure of 96% patients fell to normal levels by dynamical blood pressure observation.Moreover,290 patients with hypertension showed significantly higher control awareness rate and control rate（P0.01） after pharmaceutical care intervention.Conclusion To apply active pharmaceutical care helps improve awareness rate,treatment rate and control ate of old patients with hypertension in the community.It is an effective method to control hypertension.
