家庭医生责任制下社区糖尿病家庭分类管理效果评价

目的 评价家庭医生责任制下社区糖尿病家庭分类管理效果,为社区糖尿病管理提供依据.方法 在2015年选择龙华街道2型糖尿病患者320人为研究对象,随机分成干预组和对照组,干预组实施家庭医生制服务下糖尿病家庭分类管理模式管理1年,用SPSS 20.0统计软件对干预后两组糖尿病患者相关实验室指标,以及糖尿病健康知识知晓率、规律血糖监测率、规律日常运动率和平均每日的医疗费用支出情况进行分析.结果 干预组的收缩压、空腹血糖,糖化血糖蛋白、甘油三酯均有所下降,和对照组相比有统计学意义(P<0.05);干预组患者对糖尿病健康相关知识、态度认知以及健康行为依从性和对医师服务满意度均比对照组高,差异有统计学意义 (P<0.01);平均每日医疗费支出干预组低于对照组,差异有统计学意义(P<0.01).结论 社区糖尿病患者通过家庭医生责任制下的糖尿病家庭分类管理,能够提高糖尿病患者健康知识知晓率、健康行为依从性和对医师服务满意度,并随着糖尿病患者良好生活方式的形成使得糖尿病病情能够得到良好的控制,从而降低医疗费用支出.

Evaluation of Community Diabetes Family Classification Management under the Responsibility System of Family Doctors

Objective To evaluate the effect of community diabetes management under the responsibility system of family doctors, and to provide the basis for community diabetes management.Methods 320 patients with type 2 diabetes mellitus in Longhua Street were randomly divided into intervention group and control group in 2015.The community classification management of diabetes under the responsibility system of family doctors had been applied in the intervention group for one year.SPSS20.0 statistical software was used to compare the two groups of patients with diabetes and diabetes-related laboratory index, the awareness rate of health knowledge, regular blood glucose monitoring rate, regular daily exercise rate and average daily medical expenses.Results The biochemical parameters of intervention group including the systolic blood pressure, fasting blood glucose, glycosylated glycoprotein and triglyceride had been effectively controlled, which were significantly different from control group (P<0.05).The knowledge, attitude, health behavior and satisfaction of patients in the intervention group were higher than those in the control group, which were significantly different between the two groups(P<0.01).The average daily medical expense of the intervention group was significanthy lower than the control group(P<0.01).Conclusion Community diabetes classification management under responsibility system of family doctors can improve the health awareness of patients with diabetes, their compliance and satisfaction with doctors` service, and with the formation of good lifestyle.Diabetic patients can get better control of their glucose and reduce their medical expenses.
