伊布利特转复阵发性和持续性心房颤动的疗效与安全性评价

目的评价伊布利特转复阵发性和持续性心房颤动(房颤)的有效性和安全性。方法入选43例持续时间<90d的阵发性和持续性心房颤动患者,20例静脉注射伊布利特进行转律,23例应用普罗帕酮转律。比较两组药物转复心房颤动的成功率、转复时间和不良事件。结果伊布利特组转复房颤的总成功率显著高于普罗帕酮组(70%vs39.1%,P<0.01),阵发性和持续性房颤组的转复成功率亦分别高于普罗帕酮组(76.9%vs50%,57.1%vs27.3%,P<0.05)。伊布利特组的转复房颤时间明显缩短(34.62±4.47)minvs(62.74±12.73)min,P<0.01],未发生有临床意义的药物致心律失常作用。结论伊布利特转复阵发性和持续性心房颤动优于普罗帕酮,并有良好的安全性。

Efficacy and safety of ibutilide for conversion of paroxysmal and persistent atrial fibrillation

Objective To evaluate the efficacy and safety of ibutilide for conversion of paroxysmal and persistent atrial fibrillation.Methods 43 patients with atrial fibrillation duration less than 90 days were included into propafenone or ibutilide group.23 patients were intravenously administrated with propafenone and 20 patients with ibutilide separately.Conversion rate,times needed to converse and adverse effects were compared between two groups.Results The total conversion rate of ibutilide group was significantly higher than that of propafenone group(70% vs 39.1%,P<0.01).Both paroxysmal and persistent atrial fibrillation group patients showed higher conversion rate in ibutilide group(76.9% vs 50%),(57.1% vs 27.3%),P<0.05].Times needed to converse was significantly shortened of ibutilide group compared with propafenone group(34.62±4.47)min vs(62.74±12.73)min,P<0.01].No clinical significant adverse effects were observed in ibutilide group.Conclusion Intravenous administration of ibutilide in cardioversion of paroxysmal and persistent atrial fibrillation is more safe and effective than propafenone.
